The Community Connections Program provides short-term support to people who experience barriers finding a home and need help accessing other services to find and keep a long-term rental.
This may include helping them access health services, mental health support, getting a myGov account, applying for the NDIS and much more.
The CCP supports people who:
– are residents in the Shires of Campaspe, Central Goldfields, Greater Bendigo, Macedon Rangers or Mount Alexander
– are 20 years of age or older
– have multiple or complex needs
– are homeless, or living in low-cost or insecure accommodation
– are socially and/or geographically isolated from their community
– need help accessing health, housing, or community support services
– require case management to navigate the social services system.
How does the program work?
This is an outreach program where Haven Home Safe (HHS) support workers will travel (within the regions listed) to meet with clients where they feel most comfortable – in their home or local Community House.
Our staff work with clients to understand their needs and address any barriers to finding a home, helping them connect with the supports and services who can best assist them.
