Our case coordination services offer a range of communication and collaboration services. We work with other community service organisations in your local area to provide you with the support you need, depending on your circumstances.

If you’re looking for specific support services for yourself or to refer someone to, then this is the list of current case coordination programs we run at Haven; Home, Safe.

CCP (Community Connections Program)

Provides assistance to people who experiencing/at risk of homelessness or live in low quality accommodation (not managed by Haven; Home, Safe) and have multiple unmet health needs.

Who is it for?

People aged 20+ years who are experiencing or are at risk of homelessness, or living in low cost or insecure accommodation, who have multiple unmet health needs

Where do we offer it?

Loddon, Mallee

How do I access it?

You can self-refer or another agency can refer you to us (such as local government agencies, community services, Centrelink, Office of Housing).

Housing Access Program

Improves the capacity of the Bendigo Adult Community Mental Health Team (BACMHT) lead clinicians to identify and support the housing needs of their clients.

Directly supports clients of the adult clinical community mental health services to obtain and maintain safe, secure and affordable housing options.

Who is it for?

People aged 25-64 who are receiving community-based case management services through the Bendigo Adult Community Mental Health Team (BACMHT)

John Bomford Centre, Bendigo

Referrals for registered client of (BACMHT) only

Housing Direct

A pathway from homelessness to secure housing. Housing Direct is a specialised 2 year program assisting families and individuals achieve their self-identified goals with the aim of maintaining affordable and ongoing tenancy.

Who is it for?

Families or individuals who are homeless.

Where do we offer it?

Loddon, Barwon

How do I access it?

Referrals from specialist homelessness services only.

ICMI (Intensive Case Management)

Provides support to people who are experiencing or at risk of homelessness who have high and complex needs and need support to stabilise their personal circumstances, maintain housing or assistance out of homelessness.

Who is it for?

Families or individuals who are 18+ with high and/or complex needs who are homeless or at risk of homelessness.

Echuca

Tenancy Plus

Tenancy Plus aims to prevent homelessness and support tenants to stay in housing. Tenancy Plus used to be known as the Social Housing Advocacy Support Program (SHASP).

Tenancy Plus support providers will work with you to create a support plan that is tailored to your needs and goals. We can also connect you to a range of other support services in your area.

There are three components of the program:
  1. establishing new tenancy
  2. tenancy at risk (due to property condition, anti-social behaviour or other breach in place
  3. advocacy

Who is it for?

Tenancy Plus is for public and social housing tenants only.

Where do we offer it?

Mildura

How do I access it?

You can self-refer (if you meet the criteria of being a public or social housing tenant) or get a referral from DHHS Housing or other community-based services.

SFAR (Supporting Families at Risk)

Provides intensive support to families exiting the homelessness service system into long-term accommodation with the aim of establishing and maintaining their rental property.

Who is it for?

Families at risk of homelessness and women and children who are experiencing family violence.

How do I access it?

Referrals are taken from our Initial Assessment Planning program or other community-based services. 

Homelessness Service - Homeless Case Coordination Team

Provides case coordination support to people who are homeless or at risk of homelessness, to overcome identified barriers with the aim of securing and establishing affordable, appropriate housing.

Our case coordinators provide support to clients through the development of a case plan, which involves individualised goal setting across the different life domains.

Who is it for?

Families and individuals 18 years and over who are homeless or at risk of homelessness; who are in receipt of a Centrelink benefit; and who present with multiple unmet life barriers they wish to address.

Referrals are taken from our Initial Assessment Planning program or other community-based services.
