Swan Hill – HeyYou-T

The HeyYou-T is the Swan Hill street based outreach, it is a safe space for you to have a chat with us.

This is a mobile, street-based service and a safe space where rough sleepers can connect with our Homeless Assertive Outreach Response team.

 

The HeyYou-T carries essential items and stops at several known locations across the Swan Hill region.

The ute is carries warm food, hot drinks and as well as cold drinks.

The HeyYou-T is contactable Mon-Fri 9am- 5pm, please call 1300 428 364. 

 

If you need after-hours homelessness support please call Victoria’s statewide after-hours service (St Kilda Crisis Centre) 1800 825 955. 

The HeyYou-T client care team provides:

  • Early intervention to prevent homelessness
  • Access to emergency accommodation
  • Access to specialist services such as mental health, drug and alcohol support, allied health and more.
  • Case management to support the complex needs of people experiencing homelessness & sleeping rough
  • Early intervention strategies, aiming to reduce the psychosocial impact of rough sleeping
  • Ongoing support offering for people to be relocated off the street and into safe and permanent housing
  • Innovative responses and support to people who may not engage with traditional services
